Based on research conducted at Dartmouth around goal attainment with cancer survivors, we translated findings from those studies into health fair materials appropriate for our catchment area in New Hampshire and Vermont. Materials include information for a health fair poster/table, as well as a worksheet handout. These materials were initially used in October 2016 at a health fair and conference in Vermont and were well-received by the audience that was largely white, rural, and older. The handout was used in conjunction with oral instructions and has not been tested as a stand-alone intervention.
